Patient Instructions

Thank you for choosing our practice! In order to better serve you, please be advised of the following:

SCHEDULING INSTRUCTIONS

  • We do require a referral from your treating physician, as we are a specialist office.
  • Please have your physician office call to schedule your appointment, or have your physician call or fax a request / referral to us so that we may contact you to schedule.
  • Please have your referring physician fax over your medical records, or have them provide them to you, so that you can bring to your first appointment.

FIRST VISIT INSTRUCTIONS

  • All new patients will be scheduled an appointment in the office to meet with your physician, prior to any procedure being performed. We feel it is important that you have a chance to meet the physician and ask any questions, as well as the physician be able to have the chance to perform the necessary physical exam and review of history. If you and the physician decide to proceed with a procedure, it will be scheduled as quickly as possible. This may or may not be the same day as your examination, depending on your insurance carrier's requirements and the facility schedule availability.
  • Please arrive 30 minutes prior to your appointment in order to complete all paperwork.
  • Remember to bring your insurance card and drivers' license.
  • We DO NOT bill for all co-payments, patient co-insurances, and un-met deductibles. Please be prepared to make payment at time of service.

PROCEDURE INSTRUCTIONS

  • All procedures will be scheduled at a licensed facility (either surgery center or hospital), in order to provide the safest, most effective care possible. All efforts will be made to schedule your procedure at a facility that participates with your insurance.
  • Make plans to have somebody to drive you home after your procedure. The driver must arrive with you at the time of your appointment and be present at time of check-in.
  • Please arrive 30 minutes before scheduled appointment time in order to complete all paperwork and pre-procedure preparations. This is in order for your procedure to start on time, as scheduled.
  • Take all regular medications as prescribed. If taking coumadin or any type of blood thinners, you will need to contact your prescribing physician for approval to discontinue the medication 5 days prior to your procedure.
  • Please eat a light meal and have liquids up until procedure time - don't starve yourself.
